Early Orthodontic Treatment

  • The American Association of Orthodontists recommends an orthodontic evaluation at age 7. The reason being is that many of the permanent teeth are erupting at this age and the bite is starting to develop. An orthodontic evaluation will help determine if there are any serious problems developing. If so, treatment at this age may be easier and simpler than waiting until later when the problem may become more complicated and more difficult to treat.
    Issues like large skeletal discrepancies, missing permanent teeth, severe crowding, ectopic eruptions, impacted teeth, and crossbites can be addressed in early orthodontic treatment. If treatment is not recommended yet, at least the orthodontist and the parents are aware of any issues that will need to be addressed in the future and an appropriate treatment plan can be made.
    Problems like skeletal jaw discrepancies must be taken care of using growth modification. This type of treatment is best done while the patient is young and still has a fair amount of growth left. Other problems like missing permanent teeth and crossbites can lead to more serious complicated problems if not addressed early.

Preventing Decay While Wearing Braces-www.drgeorgebardawil.com


Having braces can present some new challenges when it comes to oral hygiene. Preventing tooth decay can be a big challenge simply because of the tendency for braces to trap food under the wires and between the teeth and the brackets. Here are a few tips to keep your teeth healthy while wearing your braces:

1. Eat Braces-Safe Foods.
Keeping your teeth from decay starts with a proper diet. Foods that are high in sugar or starch can cause more plaque which is difficult to remove during your brushing. There are certain foods that should be avoided while wearing your braces. First, sticky foods like caramel or gum can get stuck in your braces and be difficult to remove during brushing. Next, hard foods such as nuts and candy could bend wires or even break a bracket. Foods that are firm or hard to bite into like apples, carrots, or corn on the cob should be avoided. As much as we like to snack on them, those crunchy treats can harm your braces. Things like chips, ice, popcorn can also bend or break your braces. On the other hand, bananas, mangoes, milk, water, poultry, and pasta all tend to be low in enamel-busting acids.

2. Proper Brushing.
You want to place your toothbrush at a 45-degree angle against the gums in order to clean the whole tooth, and brush gently in the area between the wiring and the teeth. Use a softer toothbrush with fluoride paste for best results. Rinsing every day will help, too. Rinsing is important regardless, but especially important when you have braces as you need to disinfect the entire mouth, including those spots under the braces where your brush can’t always reach.

3. Ask About Special Cleaning Tools.
There are also special brushes, or other tools, to get under and clean your braces. You can also find many of these items at your local pharmacy.

4. Regular Teeth Cleaning.
It’s important to keep your routine appointments with your dentist and dental hygienist for a thorough cleaning twice a year or as directed. The exact frequency of these visits will be up to your dentist as some types of braces are more demanding of a regular cleaning than others.

As long as you practice good oral hygiene and follow these basic tips, you should have no problem keeping your teeth from decaying while you wear braces.

LIFE AFTER BRACES

When the day comes that your braces are removed, celebration is definitely in order! But while it’s a relief to finally have your teeth free of all those wires and brackets, you do need to take steps to ensure they stay in their new position. If you fail to use your orthodontic retainer properly, your teeth will slowly begin to revert to their previous position, erasing all the time and effort you put into correcting them by wearing braces.

Orthodontic retainers should be worn for a lifetime. While this may seem overwhelming, understand that after six months of continuous, 24/7 use, most people are able to switch to wearing their retainer only at night. After an additional 4-6 months of night wear, you may be able to start wearing your retainer for only a few nights per week. The frequency of retainer use after the initial retention period will depend on how stable your teeth become, and will be determined by your orthodontist.

Be warned that if you stop wearing your retainer all together, even after years of consistent use, your teeth may begin to shift back to their old position. Most people would admit that wearing their retainer at least a few nights each week is worth the effort in order to make sure their smile stays healthy and beautiful.

Crowding is the lack of space for all the teeth to fit normally within the jaws. The teeth may be twisted or displaced. Crowding occurs when there is disharmony in the tooth to jaw size relationship, or when the teeth are larger than the available space. Crowding can be caused by early or late loss of primary teeth, or improper eruption of teeth.

Crowding should be corrected because it can:

Gingivitis (gingiva: gums,-itis: inflammation) is a form of periodontal disease in which the gums around the teeth become inflamed and swollen. Gingivitis is commonly due to plaque (sticky bacteria and food) and tartar (hardened plaque) on the teeth. The bacteria and toxins they produce cause the gums to become infected, swollen, tender, and bleed.
 Because gingivitis can be very mild, you may not be aware that you have the condition. But it's important to take gingivitis seriously and treat it promptly. Gingivitis can lead to much more serious gum disease (periodontitis) and eventual tooth loss.

Why do I care if my teeth are crooked?

You are no longer as young as you used to be. You are past that age when everyone around you wanted to wear braces. Does it really matter that your teeth are crooked?  Is it all about looks, or there is more to it?
According to the study done by the researchers at United Medical and Dental Schools of Guy’s and St Thomas’ Hospitals, London, UK and Dental School,   there is a relationship between irregularity of  teeth and periodontal disease.
The study involved 201 children aged 11-14 years. The upper and lower incisor teeth were evaluated  for spacing, degree of crowding and overlapping. The researchers found that there was a direct relationship between the number of  contacts of teeth out of alignment and teeth overlapping each other (a more severe crowding) and a number of sites with redness of the gums. The study concluded that the results indicate that overlapping of incisor teeth is directly related to gingivitis, and this relationship cannot be explained simply by an effect of oral hygiene.http://www.drgeorgebardawil.com/

So now you have red gums that bleed.  So what?  May be your parents had bleeding gums, may be their parents had bleeding gums. Nothing new there.  You think to yourself  it is your genes.  Never mind that you do not floss, and may be brushing once or twice a week. It is your genes and there is nothing you can do about it.  It is genes’ fault and they are the ones to blame.
But that is not what this post is about.  Usually red and bleeding gums indicate that what you have, and your parents and grandparents have (or had, if they have no teeth left)  is gingivitis or inflammation of  gums.  According to the American Academy of Periodontology,  and I quote “ Untreated gingivitis can advance to periodontitis and eventually lead to tooth loss and other health problems.”
Now,  that is more than just looks.  Of course, there are other factors that could cause gingivitis,  periodontal disease and bone loss.  Let’s not forget about dental plaque.   But that is a topic for a different discussion that I will address at another time. This post, however,   is about crowding,  and how it may be related to gingivitis, potential periodontitis and subsequent bone and tooth loss.  And if you believe that crowding may lead to gingivitis,  periodontitis and bone loss,  you should be wondering  by now what , if anything,  you can  do about it.
Of course you can,  if suddenly feeling much younger,  join the crowd of  those who are looking forward to wearing those status setting metal brackets. On the other hand,   if you  think  you are mature enough to join the other crowd, then you might settle for something less obvious but, in my opinion more attractive, called Invisalign.




There are several possible causes for gum recession:
  • Abnormal tooth position, such as tooth crowding, giving inadequate cover of one or more teeth by the jaw bone.[1]
  • Hereditary thin, fragile or insufficient gingival tissue.[1]
  • Overaggressive brushing,[1] which causes the enamel at the gum line to be worn away by scrubbing the sides of the teeth in a washboard fashion
  • Periodontal disease[1]
  • Inadequate brushing or flossing, which allows bacteria to build up between the teeth, resulting in enzymes eating the bone away from the teeth
  • Eating disorders, from self-induced vomiting.
  • Grinding of the teeth (bruxism)
  • Intentional gingival retraction. For example, the adult tooth may not grow out of the gum, and to remedy this, a procedure called an exposure is done. It involves the gum tissue being cut open to allow the adult tooth to grow out. This is a less common cause of gum recession.

Dangers of gum infection

The immediate danger of gum infections is of course the threat to your oral health, and the potential for loss of teeth if it is not remedied in time. However, more and more research points to other serious health conditions that can arise from the entry of these harmful oral bacteria to the blood stream. You may not notice the effects on the rest of your body at first, but recent studies have linked gum infections to conditions such as:

  • Heart disease
  • Diabetes
  • Stroke
  • Cancer
  • Respiratory disease
  • Premature births

Underbite and Anterior Crossbites

Ideally, top incisor teeth should overlap bottom incisors vertically and horizontally. In some individuals, one or more of the top teeth are “behind” the bottom teeth upon teeth closure. This is referred to as anterior crossbite or underjet (if all incisors are affected).
Anterior crossbite, even on a single tooth can be very destructive and should be corrected as soon as they are detected.  If left untreated, they may result in damage to the structures supporting the tooth causing looseness of the tooth, bone loss, tooth fractures and/or gum recession. If anterior crossbites left untreated it may lead to muscular spasm,ear ache,Tempero mandibular joint disorder,etc

If you ever sustain damage to your braces and need to call Dr.Bardawil we can help you more effectively if you can tell us exactly which piece is in trouble! Here’s a handy diagram and corresponding list of all the parts that make up your braces.

Elastic Tie: Tiny rubber band that fits around the bracket to hold the archwire in place.
Archwire: The main wire that acts as a track to guide the teeth along. It’s changed periodically throughout treatment, as teeth move to their new positions.
Loop in Archwire: Frequently used for closing space left by an extraction. Many archwires don’t have a loop.
Bracket: Small attachment that holds the archwire in place. Most often, a bracket is cemented directly onto the tooth’s surface, eliminating the need for a band.
Headgear Tube: Round, hollow attachment on the back bands. The inner bow of the headgear fits into it.
Coil Spring: Fits between brackets and over archwire to open space between teeth.
Tie Wire: Fine wire that is twisted around the bracket to hold the archwire in place.
Band: A thin ring of metal fitted around a tooth and cemented in place. The band provides a way to attach the brackets to the tooth.
Hook: Welded or removable arm to which elastics (rubber bands) are attached.
Elastic (Rubber Band): Small rubber band that is hooked between different points on the appliance to provide pressure to move the teeth.

Before becoming an orthodontist Dr.bardawil started out in dental school and earned a DDS (dental degree), just like your dentist. After dental school however, Dr.bardawil decided he wanted to learn more about orthodontics, which is one of nine specialties within the dental field. So he stayed in school for a few more years and became an expert in orthodontics; which focuses on tooth and jaw alignment and bite problems like overbites and under bites.
Other dental specialties you may have heard of include endodontic (focusing on the soft tissue inside your teeth), periodontic (focusing on the gums and other tissues surrounding your teeth) and prosthodontic (focusing on restoring and replacing damaged teeth).
